About Me Health Passport

suffolk-disability-action-group-logoThe Suffolk Disability & Health Action Group has been working with disabled people to produce ‘About Me’ (previously described as a Health Passport), a document that disabled people and/or their family carers can complete to give health and care professionals information about themselves to help those professionals provide better care.

Endorsement of the use of the ‘About Me’ initiative is being sought from the Clinical Commissioning Groups in Suffolk, Acute and Community Hospitals, Community Healthcare Services and GPs in Suffolk in order to try to improve continuity of care between organisations.

‘About Me’ documents will be owned and kept up to date by disabled people and/or their family carers and they can be shared with many health and care professionals including Consultants, GPs, nurses, dentists, and paramedics and home carers.
